Battlefield Acupuncture Explained

Developed by Dr Richard Niemtzow of the US Air Force, Battlefield Acupuncture is a specialised auricular (ear) acupuncture protocol. Five specific points on each ear are stimulated with tiny semi-permanent ASP needles, interrupting pain signals travelling to the brain and giving drug-free pain relief.

Scientific Evidence

Peer-reviewed clinical studies supporting the effectiveness of Battlefield Acupuncture for pain management.

Low Back Pain in the Emergency Department Fox et al., 2018, American Journal of Emergency Medicine Design: Pilot randomized trial (n=30) comparing standard care vs. standard care + BFA for acute low back pain in the ED.
Findings: BFA group had significantly lower post‑intervention pain scores (NRS 5.2 vs 6.9; p=0.04). No acupuncture‑related adverse events were reported, supporting short‑term safety and suggesting efficacy for acute low back pain.
Large VA Cohort: Routine Clinical Care Zeliadt et al., 2020, Medical Care Design: Cross‑sectional cohort using EMR data from 11,406 veterans receiving BFA at 57 VA centers.
Findings: >75% had immediate pain decreases; ~60% achieved a minimal clinically important reduction. Complications were monitored; the paper notes BFA was effective across diverse patients in routine care, with no major safety signal described.
Acute Sore Throat Randomized Trial Moss & Crawford, 2015, J Am Board of Family Medicine Design: Pragmatic RCT (n=54) of BFA + standard therapy vs standard therapy alone for acute sore throat.
Findings: BFA produced significantly lower pain scores at 15 minutes, 6 hours, and 24 hours, and reduced analgesic use up to 48 hours. No clinically important adverse events were reported, indicating short‑term safety and analgesic benefit.
Emergency Department Acute Musculoskeletal Pain (BFA‑Based Protocol) Eucker et al., 2024, Annals of Emergency Medicine Design: Pragmatic adaptive RCT (n=236) comparing auricular acupuncture (based on BFA protocol) + usual care, peripheral acupuncture + usual care, vs usual care alone for acute musculoskeletal pain.
Findings: Pain reduction at 1 hour was 1.6 NRS points greater with BFA‑style auricular acupuncture than usual care (95% CI 0.7–2.6). Side effects were described as typically mild and transient, consistent with overall acupuncture safety data.
VA Implementation and Effectiveness Overview Walker et al., 2016, Nursing Outlook Design: Descriptive/implementation article within DoD/VA systems.
Findings: Reports effective use of BFA for polytrauma, PTSD‑related conditions, and musculoskeletal pain in military populations, emphasizes its non‑pharmacologic nature, and does not report serious safety concerns when administered by trained personnel.

Contraindications

Patients should NOT receive BFA if any of the following apply:

They are pregnant or might be pregnant
They have an aversion to needles or a vasovagal response to needles
They have an active infection present in either ear
They have any bleeding disorders or take anti-coagulant medication
They have current or previous keloid scar formation on either ear
They are not currently experiencing pain
